Children: 4 years of age through Grade 6

Please download the blank VBS form to your desktop.
Open it and either
1) click to fill out the editable areas, resave to desktop and attach it to email: FBCnewtonfalls@embarqmail.com or
2) simply print off, fill out and bring with you to register your child.
